8. This form is to be utilized by a commercial business seeking commercial recycling service from the City of Bay City. Please provide the following information as applicable. After the form has been received, the requested number of recycling carts will be scheduled for delivery on or by the next scheduled service day. If you are already using a blue City cart labeled with Recyclables Only stickers and want to continue to receive recycling service, please complete the form below. Your existing account will be adjusted to reflect the new recycling fee.

27. By submitting this form, you agree to the following terms: All recycling must be contained in the City provided 96-gallon cart(s). Any recycling outside the carts shall be deemed a nuisance, handled and invoiced to you. Pick up will consist of one time per week on your designated collection day. No unacceptable materials or mixed trash in the recycling cart.
29. The commercial recycling service fee is $10.00 per month, billed for a minimum of 12 months, and includes one 96-gallon wheeled recycling cart. The service fee for each additional 96-gallon wheeled recycling cart is $10.00 per month.
